Because the survey prize will be sweepstakes entries, anyone who takes the survey will need to meet these requirements:
- Be a legal resident of the United States, Washington D.C., Puerto Rico, Canada, United Kingdom, Ireland, Germany, Denmark, Honduras, or Korea.
- Be at least 18 years of age, or be 20 years of age if you are from Korea.
- Do not be an employee or the parent, child, sibling, spouse, or housemate of any employee at Zaxby’s or any affiliates.
- Possess a receipt that contains a survey invitation.
SURVEY PRIZES & BENEFITS
Zaxby’s rewards people who fill out the online Zaxby’s feedback form with sweepstakes entries. After taking the survey, you can register for a chance to win a few different prizes. Each survey taker will be given ten entries into the daily sweepstakes for the day you took the survey. If one of your ten sweepstakes entries is selected, you will win $1,000. Zaxby’s also lets you register for a weekly survey where three winners are selected from the pool of all people who took the survey that week. Each of these three winners receives $500.

- Answer all the questions about how satisfied you were with your recent trip to Zaxby’s. You will be asked about employee friendliness, speed of service, restaurant cleanliness, taste of food, and other aspects of Zaxby’s customer satisfaction.
- Submit the survey after providing answers to all survey questions.
